Scheduling & Cancellations
Your initial phone consultation is free of charge. Whenever possible, I am committed to offering sessions within a week of requested services. I also do my best to offer flexible hours for daytime and evening sessions to accommodate the varying schedules of my clients.

If you are unable to keep a scheduled appointment, a minimum
of 24 hours notice is required. Fees are due in full for any cancellations made with less than 24 hours notice.

Fees & Payment Policy
Please feel free to contact me if you would like information regarding my fee schedule. Payment, or co-payment, is due in full at the time of each session. I accept checks, cash, and credit cards. Each check returned due to insufficient funds results in an additional fee of $25.

Consultation outside of session that exceeds fifteen minutes will be billed incrementally at my hourly rate. This includes, but is not limited to, collateral contacts with family members, courts, clinics, or schools either in person or via telephone, text, or email correspondence.

A collection agency may be used for accounts that are 60 days past due. Confidentiality regarding financial information is waived when submitting for insurance or to a collection agency.

Insurance
As a courtesy, I will assist you in obtaining mental health benefits from the providers listed below but you are responsible for determining your coverage for mental health services. Prior to our first session, please check with your insurance provider to determine the parameters of your mental health coverage. Be sure to inquire about the following:

1) deductibles, 2) co-payments, and 3) session limitations

I am a contracted provider for the following insurance companies:

MDwise Marketplace

Sagamore
Southeastern Indiana Health Organization (SIHO)
Cigna

Out-of-Network Coverage
If you would like to file for out-of-network benefits from a different insurance company, I will prepare a monthly statement that you can submit with your claim. You would need to sign an Insurance Claim Supplement Form authorizing me to provide appropriate information to your insurance provider.
